Job Overview:
We are seeking an experienced and strategic Group Director of Case Management to lead multi-site case management operations across healthcare facilities in Modesto, CA. This executive-level position will drive best practices in care coordination, utilization management, and discharge planning while supporting operational efficiency and regulatory compliance across the system.
Key Responsibilities:
Provide leadership and oversight of case management services across multiple hospitals
Ensure alignment with organizational goals, compliance standards, and patient care objectives
Lead business planning and strategic initiatives to optimize performance and reduce unnecessary utilization
Collaborate with executive and clinical teams to enhance care transitions, patient outcomes, and interdisciplinary communication
Manage team performance, provide mentorship, and promote professional development
Required Qualifications:
Bachelor’s degree in Business, Nursing, or Health Care Administration for RNs or a Master’s in Social Work (MSW)
Active RN or LCSW/LMSW license in the applicable state(s)
Minimum of 5 years of leadership experience in acute hospital case management or a healthcare setting
Strong organizational, communication, and leadership skills
Proficient in critical thinking, problem-solving, and computer systems
Preferred Qualifications:
Advanced degree in business, nursing, or healthcare-related field
Accredited Case Manager (ACM) certification
Experience in multi-site case management, business planning, or project management
