A well-established senior care organization in San Bernardino, CA is seeking an experienced Physical Therapist to deliver high-quality rehabilitative services to a primarily elderly patient population. This role focuses on improving mobility, functional independence, and quality of life through individualized treatment plans in a clinical care setting.

Key Responsibilities:

Evaluate patient conditions and develop personalized physical therapy treatment plans
Deliver rehabilitative care to restore physical function and manage pain
Monitor progress and adjust therapy interventions as needed
Document patient evaluations, treatments, and outcomes accurately
Collaborate with interdisciplinary team members to support integrated patient care

Required Qualifications:

Bachelor’s degree in Physical Therapy with a minimum of 5 years of experience
Valid California Physical Therapy License or proof of eligibility
Degree from outside the U.S. must be evaluated for U.S. equivalency
2 years of clinical experience with a frail or elderly population preferred
CPR certification preferred and must be maintained
