Job Overview:
We are seeking a dedicated and experienced RN Care Manager to join a dynamic healthcare team in Tarzana, CA. In this role, you will coordinate and manage patient care across acute, ambulatory, and post-acute settings, supporting quality outcomes, appropriate resource utilization, and smooth transitions across the continuum of care.

Key Responsibilities:

Conduct comprehensive assessments and develop individualized care plans
Coordinate and facilitate interdisciplinary care team meetings and discharge planning
Perform utilization management and review for medical necessity and appropriate levels of care
Collaborate with patients, families, physicians, and other care providers to optimize care outcomes
Ensure compliance with care management policies and regulatory standards

Required Qualifications:

Bachelor’s Degree in Nursing or commitment to obtain a BSN within 3 years of hire (if applicable)
California RN license upon hire
California Fire and Life Safety Card within 30 days of hire
2 years of experience in a healthcare setting (acute, ambulatory, or post-acute)
2 years of case management experience (care coordination or utilization management) or successful completion of a Care Management Transitions in Practice (TIP) program

Preferred Qualifications:

Master’s degree in Nursing or a healthcare-related field
Certification in Case Management (ACM or CCM) upon hire
